Expand Hospital at Home

A Philanthropic Opportunity at the Penn Center for Health Care Transformation & Innovation

Many patients spend days in the ER and hospital for conditions that could be safely managed at home, if they had coordinated monitoring and support from their care team.

Through multiple pilot efforts, CHTI aims to do just that: building and expanding upon a model of high-quality, acute health care in the home setting, grounded in compassion, community, and the same clinical excellence that defines Penn Medicine today.


In a pilot study, CHTI demonstrated that home care could reach patients at the right place and right time – as 90% of patients who used the program avoided being admitted to the hospital, and 82% avoided another emergency visit within a month after enrollment.


Bringing hospital-level care directly into patients’ homes will improve healing, lower costs, and build trust with those we serve. Your gift can expand this program’s pilot and help build a model of what care at home can look like at Penn Medicine and beyond.








$50,000: support our team in leading research through the care at home pilot, evaluating patient and caregiver experiences, testing new ideas as they emerge, and sharing timely insights at Penn and nationally.

$100,000+: establish a patient assistance fund to support essential needs that fall outside the boundaries of traditional reimbursement, including transportation for safe discharge or follow-up care, temporary, in-home aides for patients with mobility limitations, and so much more.

$150,000: launch a video education program for patients and families receiving care at home. In an increasingly digital world, health-related videos provide an especially engaging way to deliver information – everything from sling positioning or troubleshooting health monitoring equipment, to continuity of care and symptom management tips.

$235,000: invest in a Patient and Family Care Navigator, a steady and compassionate guide to help patients and families navigate the complex transition from hospital to home setting and ensure care plan continuity.
