CHTI harnesses artificial intelligence (AI) to help free care providers from administrative burdens and streamline treatment in ways that offer patients faster, safer, more personalized care.

For patients and clinicians, this can mean less time spent on paperwork, easier access to appointments, or pinpointed patterns in each patient’s data — turning complexity into clarity, data into action, and innovation into impact.

By supporting CHTI’s AI-driven initiatives with a gift of any size, you help build a smarter, more connected health care system.

The High-Risk Follow-Through initiative uses AI to flag patients at high risk of missing follow-up care after important findings such as lung nodules or abnormal imaging. AI models assess risk using clinical, demographic, behavioral, and social determinant data, and trigger follow-up workflows to close gaps in care.

  • Why AI? Traditional tracking systems are reactive and often overlook subtleties. Using AI, care teams can better identify which patients need follow-up because the models rapidly sift through clinical notes and other records that are otherwise difficult to review.
  • Impact: The program helps prevent diagnostic delays and supports timely, equitable care, especially for vulnerable or hard-to-reach populations.

Chart Hero is a new project that aims to deliver crucial patient information to a clinician’s fingertips. Using large language models (LLMs), a type of AI, care teams will be able to quickly and easily generate summaries based on information stored in a patient’s electronic health record. In the long-term, we envision creating a chatbot that can nimbly assist clinicians in gathering and synthesizing patient data.

  • Why AI? Medical charts contain loads of data stored across tabs and documents. AI can efficiently extract and summarize information that would take much more time and effort for a person to do.
  • Impact: Chart Hero has the potential to reduce the cognitive burden on clinicians and free up time so they can focus on the hands-on aspects of patient care.
