How CHTI Drives Change

A Philanthropic Opportunity at the Penn Center for Health Care Transformation & Innovation

CHTI takes on high-priority challenges in care delivery that have no clear playbook.

With our adaptive, system-aligned approach and multidisciplinary team – which includes clinical experts, project managers, engineers, data scientists, and human-centered designers – we deliver rapid transformation that sticks.

We concentrate our work on two domains to advance care delivery:

  • Our Emerging Care Delivery division reimagines paradigms for when, where, and how care is delivered. Current programs in this portfolio are exploring how we can effectively provide hospital-level care at home, accelerate care for breast cancer patients with new offerings, and increase prescriptions of life-saving heart medication for at-risk patients through pharmacist-led conversations.
  • Our Care Delivery Augmentation division develops solutions that reduce burden and close operational gaps through a technology-informed, human-centered approach. Often these are whitespace solutions that provide crucial enhancements until a commercial product becomes available. Current programs in this portfolio include
    • An automated tool to process external medical records, which has enabled more than 12,000 hours of staff time to be reallocated to higher-value work.
    • A text message–based navigator that helps patients prepare for colonoscopies, which has led to an estimated 2,000 more colonoscopies completed per year by improving show rates. This increase in procedures can mean better health outcomes because of early disease detection and shorter wait times for colonoscopies due to fewer lost appointments.








CHTI also sponsors programs to cultivate portfolio opportunities, talent, and insights:

  • Our Accelerator Program invites employees across Penn Medicine to submit a project proposal, and we support selected project teams through design, testing, and implementation.
  • CHTI’s Catalyzer Program is a pathway for health system executives to submit and receive support on their most pressing problems in care delivery.
  • Our fellowship and internship programs provide the next generation of leaders with immersive experience in health care transformation; and enhance the diverse expertise and interdisciplinary training environment that makes innovative work possible.
  • The Transformation Science Program fosters the translation of high-impact scientific studies into real-world care delivery solutions.
